Have your insurance card available at each visit. Any applicable copays and deductibles will be due before your visit. Remember that insurance is considered a method of reimbursing the patient for fees paid to the doctor and is not a substitute for payment. Some companies pay a fixed allowance for certain procedures while other pay a percentage of the charge.

Wellness physicals that require a form to be filled out will be done on a cash basis. We accept Visa, Master Card, and American Express; as well as cash, checks and debit cards. Should you ask us to bill your copay, a service charge of $10.00 will be added.
