Have
your insurance card available at each visit. Any applicable copays
and deductibles will be due before your visit. Remember that insurance
is considered a method of reimbursing the patient for fees paid
to the doctor and is not a substitute for payment. Some companies
pay a fixed allowance for certain procedures while others pay a percentage
of the charge.

Should you question if a procedure
or service is covered by your insurance plan, please call your insurance
carrier for eligible benefits. Each plans benefits are specific
to your employer group.
If you should have questions regarding smaller insurance carriers we contract with or your
