New Policy Effective March 1, 2009
In order to serve our patients with the highest level of care we will only be refilling prescriptions during an office visit with your provider. This will allow the provider to discuss at length any changes or additional information with the patient at that time regarding your ongoing care.
Therefore, we will no longer be refilling prescriptions over the phone. Prescriptions that are mail order will need to be brought to the appointment and will no longer be faxed. Mail order prescriptions once written will need to be mailed by the patient to the mail order pharmacy. If the patient has run out of medication a small quantity will be called in until the patient can visit with their provider. However, antibiotics and narcotics will not be filled without a scheduled appointment.

Emergency & Hospital Care
We recognize that you may
have an emergency situation arise, and we will do our best to respond
to your problem promptly. In the event of a severe life threatening
situation, call 911 or go immediately to the emergency room of the
nearest hospital. Ask them to contact your physician at Warner Family
Practice. If the situation is not urgent, but you desire contact with
a doctor, please call 480-831-8457 and stay on the line to be directed
to the physician on call.
Hospitalization & Surgery
Our physicians are on staff at Banner and Chandler Regional Hospitals; however, they have a group of hospitalists to manage your care in the hospital. This group consists of internal medicine specialists who specialize in hospital medicine. If you should desire to talk to your doctor at Warner Family Practice after being admitted to the hospital, please call the office at 831-8457 and your doctor will contact you in the hospital.
No Show Fee
We try to see all patients on an appointment basis, and we request that you call 831-8457 in advance, so we may reserve a time for you. We make every effort to honor all time commitments and request that you extend the same courtesy to us. Occasionally, an emergency may arise, causing delays in our appointment schedule. We will make every attempt to inform you of any extended wait and give you the option of rescheduling at that time. Please call to advise us of any cancellation or delays, to avoid a no-show appointment fee of $50.00. In the event that you are delayed, the provider will be consulted as to whether their schedule will accommodate the delay or if it will be necessary to have you reschedule. We utilize an automated reminder system, called MedVoice, to remind patients of their appointment two days prior to their scheduled appointment. This message must be listened to in its entirety in order for it to be erased.
Checks & Non-Sufficient Funds
We will accept checks only with a credit card or debit card, the number will be written on top of the check. Returned checks will incur an additional $25 charge.
Insurance and Fees
Have your insurance card available at each visit. Any applicable co-pays and deductibles will be due prior to your visit. Remember that insurance is considered a method of reimbursing the patient for fees paid to the
doctor and is not a substitute for payment. If you question if a procedure or service is covered by your insurance, please call your insurance carrier for eligibility benefits. Each plan’s benefits are specific to your employer group. Wellness physicals that require a form to be filled out will be done
on a cash basis.
We accept Visa, MasterCard, American Express and Discover; as well as cash, checks and debit cards. Should you ask us to bill your co-pay, a service charge of $10.00 will be added. If you should have questions regarding your bill, please call 831-8457 (option 4) or go Online Payment to pay.
Care to a Minor
According to HIPAA guidlines services can not be rendered minors unless accompanied by a designated adult with my written permission or alone with a notarized note.
