We realize everyone’s life is busy and we don’t plan when we get sick, that is why we have increased our staff to meet the needs of our patients. We try to see all patients on an appointment basis, and we request that you call in advance at 480-831-8457 so we may reserve a time for you.

We make every effort to honor all time commitments and request that you extend the same courtesy to us. Occasionally, an emergency may arise, causing delays in our appointment schedule. We will make every attempt to inform you of any extended wait and give you the option of rescheduling at that time. Please call to advise us of any cancellation or delays, to avoid a no-show appointment fee of $35.00. In the event that you are delayed, the provider will be consulted as to whether their schedule will accommodate the delay or if it will be necessary to have you reschedule.

Wellness is imperative to prevent future illness and discover illness before it advances. Yearly wellness exams are encouraged for men & women. Children should be seen every 10 years to receive the required tetanus boosters. Baby immunizations are given at birth, 2, 4, 6, 12 and 15 months and prior to kindergarten. A lead screen and a CBC are recommended at 9 months of age.

Recall cards are sent on yearly pap smears, and annual physical reminders are sent during the patient’s birthday month. Patients with hypertensive, cholesterol, diabetic and/or pulmonary disorders are recalled every 3 to 6 months. Our goal is to practice preventative medicine in the patient’s best interest. Please bring your immunization records with you to each visit.

In an effort to provide the best possible care and reduce the amount of time the pre-physical appointment currently requires, we are requesting that you fill out the online health questionnaire at least two weeks prior to your pre-physical appointment. You may access the questionnaire by clicking on the image above. The questionnaire will be directly submitted to our office once completed online through a secured site.
