We request that you allow our office and your HMO insurance plan 7-10 days to process you referral for specialist appointments and with other facilities. Urgent referrals will continue to be given priority. Most visits to the specialist or testing facilities require a visit to your primary care physician (PCP) prior to issuing.

Once your doctor refers you, make an appointment with the specialist or testing facility. Confirm that the facility is contracted with your insurance. Call the Warner Family Practice referral department at 480-831-8457, press option "3" or complete the form below and they will record your appointment date and time.

The authorization will be faxed to your specialist before your appointment. A call to your specialist or testing facility the day before is advised to check that your referral has arrived properly to their office. Our office must be advised of all follow-up appointments, as new authorization must be obtained. Once again, please allow 7-10 days for this authorization to be completed.

Lab & x-ray tests are sent to an outside reference facility with the exception of the allowable in-office tests. Please allow 2-4 days for x-rays and lab results, and 7-10 days for cultures and tissue identification. Our automated voice notification system will contact you regarding your results. Follow the instructions given to you by our office prior to your visit to the lab. Call the med voice number to retrieve the message by entering your social security number or a designated patient identification number. Should you have further questions, please contact your doctor. If you are not contacted within 10 days, please feel free to call your doctor to check on results.
