Raven Hooks, PA-C, joined Warner Family Practice in 2020. She values creating individualized treatment plans and goals to help her patients live their best life.
Raven was born in northern Indiana, living there until she was ten years old. Since moving to Arizona in 2001, she has lived in Mesa, Gilbert, and Chandler. Raven played softball growing up, and most of her weekends were spent on a softball diamond, enjoying the Arizona weather. She went to Basha High School where her softball team won the state championship in 2008 and 2009.
After seeing how the shortage of medical providers affected daily life as she grew up, Raven became passionate about all people having access to high quality healthcare. She always enjoyed science, studying biology and physiology (how our cells work and manage the complex systems of our body). She later became interested in becoming a Physician Assistant after a compassionate PA took the time to breakdown an issue she was having, explaining the treatment plan in detail. That interaction motivated her to become a provider that listens to patients and takes the time to have a dialogue with them. Compassionate medicine has better health outcomes!
“The most rewarding part of practicing medicine is empowering a patient to achieve their health goals.”
Raven knows the value of placing an emphasis on preventative care. She believes that she and the patient are a team, and both need to be active participants in their health care. Raven’s job is to give every patient she sees access to tools and education to use, whether for lifestyle changes, physical therapy, or medications that will increase their quality of life and health.
When talking to Raven about your health history, know that she will listen to you with compassion and meet you where you are. She believes each patient should have individualized health care goals, that you will discuss those goals together, and she will always support you along the way.
“Success is individualizing treatment plans and goals to fit each patient. Then the patient can be an active part of their health.”
When Raven has free time, she enjoys hiking, especially getting up to the beautiful scenery of northern Arizona in Sedona, Payson, and Prescott. Her favorite hike is Horton’s Creek Trail in Payson – its shaded, luscious green vegetation and beautiful creek are alongside you the entire way. Raven enjoys watching baseball games; going to spring training games is one of her family’s traditions. She also enjoys spending time with her dog. Nightly walks are something they look forward to, as it is calming for both of them.
“I will always take the time to listen to your concerns, and I want you to feel comfortable bringing up any questions you may have. My goal is to help you live your best life!”
