Warner Family Practice continues to provide many treatments and testing in-house, meaning that many specialist visits can be completed in our office. Still, there is a need for referrals and authorizations in order for specialist visits that we do not offer.
When requesting a referral for a specialist appointment, we graciously ask that you allow our office and your insurance plan 7-10 days to process your referral for specialist appointments and/or other facilities. We do give priority to urgent referrals.
Most visits to the specialist or testing facilities require a visit to your primary care physician (PCP) prior to issuing.
Once your provider refers you, confirm that the facility is contracted with your insurance, then make an appointment with the specialist or testing facility. Call the Warner Family Practice referral department at 480.831.8457, press option 3 or complete the form on our web portal and they will record your appointment date and time.
The authorization will be faxed to your specialist before your appointment. A call to your specialist or testing facility the day before is advised to check that your referral has arrived properly at their office. Our office must be advised of all follow-up appointments, as new authorization must be obtained. Once again, please allow 7-10 days for this authorization to be completed.
